Southcoast Health to offer coordinated care for MassHealth patients
[Source: heraldnews.com] Beginning January 2018, patients who are seen by Southcoast Health primary care providers and are covered under MassHealth (Medicaid) will transition to an Accountable Care Organization (ACO) model of care.
An ACO is a program where physicians, hospitals and other healthcare providers work together to provide integrated health care for patients with the goal of improving health and containing costs.
